《柳葉刀》(The Lancet)
發表的“柳葉刀特邀重大報告:關於痴呆的預防、干預和照護”2020年報告,全球40%痴呆症可歸因於12種可調控風險因素,空氣汙染便是其中之一[1]。
雖然流行病學觀察性研究發現大氣細顆粒物(PM2。5)與其他空氣汙染物長期暴露與認知功能下降、痴呆症發病風險相關[2-11],但這類研究結果受諸多混雜因素(如社會經濟因素)的影響,難以確定PM2。5暴露與認知退化是否具有因果關係[12]。自然實驗被認為可以提供因果關係的證據(Joshua Angrist和Guido Imbens由此分享了2021年諾貝爾經濟獎),而中國政府自2013年起迅速實施的清潔空氣行動——包括《大氣汙染防治行動計劃 (2013-2017)》(以下簡稱“大氣十條”)和《打贏藍天保衛戰三年行動計劃(2018-2020)》(詳見原文附件)——正是一次檢驗PM2。5等空氣汙染物對健康影響的自然實驗。

研究方法
本研究使用2014年和2018年開展的中國老齡健康調查(CLHLS)資料,樣本來源於全國23個省、直轄市和自治區。所在省級政府基於“大氣十條”政令下設定年均PM2。5濃度降低≥5%的研究物件為干預組,其餘為對照組。本研究基於調查物件居住街道匹配調查日期前 12個月內六種常見空氣汙染物(PM2。5、PM10、SO2、NO2、O3和 CO)的平均濃度,使用簡易智力狀態檢查量表 (MMSE) 評估認知功能,使用雙重差分(Difference-in-differences)方法分析空氣汙染物濃度變化與認知功能評分變化的關聯性。(詳見原文figure 1)
研究結果
2014年至2018年期間,
干預組人群與對照組相比MMSE評分降低幅度顯著更小,認知功能衰退速度更慢,平均組間差異具有統計顯著性。研究還發現空氣PM2.5和SO2暴露水平與認知功能退化速度具有穩健的劑量反應關係。

研究結論
